Central Halifax Personalised Care Team are holding / attending a number of community engagement events, come along and say hello.

• Together Housing event, Thursday 14 March, 10-2 at Halifax Town Hall.Community Services Fair for over 55s. Social Prescribing Link Workers and Care Co-ordinators will attend to discuss Personalised Care.
